Service

Supported Community Connections — from visitor, to regular, to friend.

Support for the people living in our host homes to take part in community life: a class, a gym, a faith community, a volunteer shift. Funded through the HCBS-DD waiver when it’s in the person’s service plan. Across the Denver metro and Boulder County.

What SCC looks like

Going to the same places, often enough to be known.

Supported Community Connections (SCC) is a Day Habilitation service in Colorado’s HCBS-DD waiver. A trained Grace Mountain provider goes with the person to the places they choose, and helps them build the skills and relationships to belong there.

We think of it in three steps. A visitor goes somewhere once. A regular goes often enough that people know their name. A friend has someone there who would notice if they stopped coming. Our providers help each person move along that path at their own pace: get curious, watch and listen for how they communicate, with words or without them, and plan around what they enjoy and want to try.

Who SCC serves

People living in a Grace Mountain host home.

SCC is available to the adults living in our host homes when it is in their service plan. Their case manager adds it to the plan. The person chooses whether to use it and who provides it: their own host home provider or another Grace Mountain provider.
