The full spectrum of HCBS-DD placement needs.
Waiver
HCBS-DD only. We do not currently serve SLS or CES waivers.
Population
Adults 17+ with intellectual and developmental disabilities.
Service Area
Arapahoe, Douglas, Denver, Jefferson, and Adams counties. Grand Junction coming soon.
Specialty
Regional center transitions and clients with complex behavioral, medical, or forensic histories.
Placements other agencies pass on, we get curious about.
Our philosophy is that every behavior is an authentic need trying to be met. That changes which placements we’re prepared to take.
Behavioral complexity
Aggression, elopement risk, self-injurious behavior. We accept these referrals on a case-by-case basis with elevated training for the host home team.
Medical complexity
G-tube, oxygen, seizure disorder, non-ambulatory clients. Sandra, our Agency Nurse, oversees medication and health compliance across every home.
Forensic history
Prior offenses, including sex-offense histories, are evaluated case-by-case. Provider home selection and ongoing oversight reflect the placement context.
Dual diagnosis
IDD plus serious mental-health diagnoses (schizophrenia, bipolar). Active substance use is evaluated case-by-case.
Regional center transitions
We receive direct referrals from regional centers and institutions. Host home teams receive elevated training designed for the level of support these moves require.
Aging-in-place
Adults aging within the IDD system. We are committed to keeping placements stable as needs change rather than passing the client to a new agency.
The team behind every client we serve.
Clinical oversight
Our Agency Nurse monitors health and medication compliance across every home.
QA home inspections
Our QA specialist inspects every home in person for HUD and state compliance.
Provider training
Every host home provider completes our structured training program.
Dedicated care coordinator
Each client is assigned a Care Coordinator who owns the relationship after move-in.
Four steps from your email to move-in.
