Structured support and compensation for family members who deliver personal care to their own adult loved ones through our certified caregiver model. Funded through the HCBS-DD waiver.

What this model is

The caregiver is family. The structure is professional.

In Grace Mountain’s Family Caregiver model, a family member who provides day-to-day personal care for an adult loved one with IDD becomes a certified caregiver under our agency. They complete our training program, follow our documentation and compliance practices, and receive compensation through the HCBS-DD waiver.

We surround the family caregiver with the same support our Host Home providers get: clinical oversight from our Agency Nurse, a dedicated Care Coordinator, and ongoing training. The result is a model that honors what families are already doing while making it sustainable.

Who it’s for

Families already carrying the work.

This model fits when:

  • An adult loved one with IDD lives at home with family.
  • A family member is providing the bulk of personal care already.
  • The family wants the support of a structured agency relationship rather than navigating systems alone.
  • The client qualifies for the HCBS-DD waiver.

It’s not a fit when the family caregiver isn’t comfortable with training and documentation requirements, or when the client’s needs exceed what one family member can sustainably provide.
