How we work, in plain language.

Grace Mountain Agency is a Colorado Program Approved Service Agency (PASA). That means the State of Colorado has approved us to provide residential and in-home services for adults with intellectual and developmental disabilities who qualify for the HCBS-DD waiver.

Our work begins with curiosity. Every behavior is a real need trying to be met — and when we understand the need, the placement starts working in a way it doesn’t when behavior is treated as a problem to manage.

How we vet provider families

What is a Colorado PASA?

Arapahoe, Douglas, Denver, Jefferson, and Adams counties in the Denver metro. I-70 corridor expansion is coming soon.

What does the placement path look like?

Four steps: first conversation with Christina (our CEO), a discovery conversation about your loved one's needs and goals, a match-and-meet with the receiving Care Coordinator and host home team, and move-in with ongoing Care Coordinator support.

How is each provider home vetted?

Background and reference checks on every adult in the home, structured provider training led by our Provider Training Lead, in-person QA home inspections for HUD and state compliance, and ongoing clinical oversight from our Agency Nurse.
